Abstract

PTPN IV Regional 3 Sei Galuh Palm Oil Mill (PKS) is a manufacturing company in palm oil processing. Therefore, this company requires a very large source of energy that is used to drive machines and equipment. Boilers are machines that play an important role in the sustainability of the performance of palm oil mills that function to produce and supply steam to meet the needs of factory processes and employee housing around the factory. This study aims to evaluate the performance of boilers that often suffer damage and the frequency of damage that occurs and has an impact on the stagnation of the production process at PTPN IV Regional 3 Sei GALUH. By using the Failure Modes And Effect Analysis (FMEA) method and identifying the factors that cause damage using fishbone diagrams and providing alternative solutions in overcoming boiler damage problems that occur. The results of the study obtained the highest contributor to the down time in the production process at PTPN IV Regional 3 Sei GALUH , which was 144.09 hours with a percentage value of 48% and a total down time of 302.09 hours. Based on the analysis of fishbone diagrams, 5 factors causing damage to leaking boiler pipes were obtained consisting of humans, methods, machines, environment and systems. Alternative solutions proposed are preventive, predictive and corrective treatments
